1. Age Requirement

• You must be 19 or older to sign your own waiver.
• Minors must have a parent or legal guardian sign on their behalf.

2. Assumption of Risk

By participating in any program, class, or activity, you understand and accept that martial arts, fitness training, and use of gym equipment carry risks including:
• Injury, including serious injury or paralysis
• Property damage
• Permanent disability
• Death
You voluntarily assume all such risks.

3. Liability Release

• You release and hold harmless Fight Club Fitness, LLC, its staff, coaches, agents, and facility from all claims related to injury, loss, or damages.
• This includes injuries caused by equipment use, activities, other participants, or accidents.

4. Policies & Rules Agreement

• You agree to follow all gym rules, safety guidelines, and instructions from staff.
• Policies may be updated at any time, and you agree to follow the most current published version.
• Fight Club Fitness reserves the right to revoke training privileges for rule violations.

5. Skills & Physical Ability

• You confirm you have the physical and mental ability to safely participate.
• If unsure, you agree to ask staff before participating.

6. Emergency Medical Consent

• You authorize Fight Club Fitness staff to seek emergency medical treatment for you if needed.
• You understand you are fully responsible for any medical costs.

7. Insurance Responsibility

• Fight Club Fitness does not provide medical or health insurance.
• You are responsible for your own insurance coverage.

8. Guest Policy

• Guests are allowed when approved by staff.
• Guests must complete and sign the waiver before participating.
• Guest sponsors must remain in the facility during the visit.
• Minors must have parent/guardian consent.

9. Media & Photography Consent

• Fight Club Fitness may record photos, videos, or audio during classes or events.
• By participating, you authorize the use of your name, image, and likeness for marketing and promotional purposes.

10. Governing Law

• All agreements and waivers are governed by the laws of the State of Alabama.

11. Severability Clause

• If any portion of the agreement is found unenforceable, the remainder stays in effect.

12. Entire Agreement

• The waiver and its terms represent the full agreement between you and Fight Club Fitness.
• You acknowledge you had the opportunity to seek legal advice before signing.
